King – Queen Nomination Form for 2023 Ball

Community Conscience is requesting nominations for a prospective King and/or Queen of the 2023 Mardi Gras Ball.

The Trustees of Community Conscience will choose the King and Queen. The honorees will be recognized for their outstanding volunteer service at the annual Mardi Gras Ball on March 4, 2023.

The Community Conscience Board will determine the honorees using the following criteria:

Length of service to the agency
Commitment to the cause
Dedication and caring attitude for the services provided

One of the goals of Community Conscience is to foster courtesy, respect and human dignity in the public dealings of basic assistance agencies. We hope the individual nominated exemplifies this goal.

Nominations should be received at our office in person at 80 E. Hillcrest Drive Suite 110, Thousand Oaks or by email to robin@communityconscience.org no later than December 05, 2022.
